It's understood that three men were thrown through the windshield of the truck during the collision, with the man who was killed found below the overpass.

One of the men was trapped beneath the vehicle for more than an hour as crews worked to free him.

He was treated at the scene and taken to Liverpool Hospital.

Another man was airlifted to Westmead Hospital's trauma unit.

A woman behind the wheel of the car was flown to Sydney's St George Hospital.

All three patients were in critical conditions when taken from the scene.

Three fire crews attended the scene to cut passengers free from the vehicles.

Lawrence Hargrave Drive is closed in both directions, while Bulli Pass is closed southbound at the M1 Princes Motorway, as debris from the crash has been spread across the neighbouring road.

Motorists have been urged to avoid the area, as traffic is heavy and queued from Park Road at Bulli.

The roads are expected to remain closed over the next few hours.

Police have established a crime scene, which will be forensically examined by specialist officers from the Crash Investigation Unit.
